Constructed of off white paper with black ink, named to HAUPTWACHTMEISTER DER SCHUPO (Schutzpolizei) HERBERT WOLLITZ, of SS Polizei Regiment 16, awarding Wollitz the Black Grade Wound Badge on 6 March 1945 for a single wound suffered on 22 February 1945, certified by the handwritten signature of an unidentified Oberstabsarzt and Chefarzt of Feldlazarette (mot.) 607, measuring 14 cm (w) x 20 cm
